Jæja. Þá er loksins búið að slá því föstu: Offita er ekki átröskun. Þótt ótrúlegt megi virðast þurftu helstu sérfræðingar veraldar í alvöru að setjast niður og ræða það hvort offita, þ.e. þyngdarstuðull 30 og yfir, ætti að flokkast sem geðröskun. Að sjálfsögðu komust þau að þeirri niðurstöðu að svo var ekki, enda bæði fáránlegt og ógnvekjandi til þess að hugsa að hægt sé að greina geðraskanir með vigt og málbandi. Feitt fólk getur auðvitað þjáðst af átröskunum eins og aðrir, en það er einmitt málið: Átröskun, eins og aðrar geðraskanir, geta komið fyrir hjá fólki af öllum stæðrum og gerðum.
Ég hef rekist á þessa vitleysu á ólíklegustu stöðum. Meira að segja hef ég lesið greinar eftir virta höfunda í virtum fagtímaritum sem tala um að „offita sé sú tegund átröskunar sem hafi aukist hraðast hin síðari ár“ og „offita sé alvarlegasta átröskunin“. Kannski er verið að rugla saman offitu og ofátsröskun (binge eating disorder). Í því felst ákveðin rökvilla. Flestir sem þjást af ofátsröskun eru feitir. Rétt er það. En flest feitt fólk þjáist samt ekki af þessari röskun.
Alveg eins og allir kettir eru spendýr en ekki eru öll spendýr kettir. Langt því frá.
Já er það, „lendir“ fólk bara í þessu og hefur ekkert val. Það eru semsagt ósjálfráð viðbrögð að troða í sig mat og fitna um of?
Offita er ekki geðröskun samkvæmt þessu, en er gjarnan afleiðing geðrænna vandamála svo sem þunglyndis.
Það hefði nú samt ekki komið mér á óvart að þeim hjá APA hefði dottið í hug að flokka offitu sem geðröskun, svona í ljósi sögunnar þar sem samkynhneigð var geðröskun í DSM kerfinu fram til 1973.
Er búin að reyna að skilja þennan pistil en er líklega of illa gefin. Hvort ertu að segja að offita sé geðröskun eða átröskun og hver er munurinn?

Átröskun er geðröskun, rétt er það. Offita er á margan hátt afleiðing af geðrænum vandamálum, enda hefur oft verið bent á það. Það er t.d. eitt af einkennum þunglyndis að borða of mikið.
Ég skil ekki málflutning sem þennan. Offita er mikið heilsuvandamál og einnig sprottið af andlegum toga. Hér er ég ekki að tala um þétta einstaklinga eða aðeins þybbna.
Það eru mörg úrræði fyrir einstaklinga sem þjást af geðrænum vandamálum og eru of feitir.

Ég verð að segja eins og er að ég fatta þennan pistil ekki 100% en held samt að ég skilji nokkurn veginn hvert þú ert að fara. Offitan sem slík ein og sér er ekki átröskun (rétt skilið?) Það breytir því ekki að offita er stundum (og jafnvel oft) afleiðing átröskunar.
Mér fannst alltaf frekar hlálegt þegar ég var sem verst af minni átröskun að ég var talin vera löt, gráðug og vitlaus að geta ekki látið af hömlulausum ofátsköstum, en ef ég hefði stungið fingrinum ofan í kok eftir hvert kastið og losað mig við herlegheitin, þá hefði ég allt í einu verið með geðröskun.
Satt best að segja reyndi ég oft að æla þessu og stundum þannig að það blæddi úr kokinu á mér. Ég hef bara alla tíð átt erfitt með að æla þannig að þetta gekk ekki hjá mér. Þess vegna hélt ég áfram að vera löt, gráðug og vitlaus – en „ekki með átröskun“ 😉

Það er athyglisvert að í greininni sem þú bendir á er talað um að 45% af þeim sem greindir eru með átraskanir falla ekki undir anorexiu, bulemiu eða binge eating, þ.e. eru „aðrar ósértækar átraskanir“. Slíkt bendir til að núverandi flokkun sé ekki nógu nákvæm, en enn sem komið er virðist ekki vera búið að festa fingur á einhverja ákveðna tegund átröskunar sem gæti komið til viðbótar.
Spurningin er líka: Hvenær er eitthvað „röskun“ (sjúkdómur) og hvenær er það bara „kvilli“ eða óhollur „ávani“ eða eitthvað slíkt sem er óæskilegt en ekki sjúkdómur þó. Í greiningabók bandarísku geðlæknasamtakanna er oft miðað við að ákveðin hegðun sé orðin að röskun ef að hún er síendurtekin yfir langan tíma og veldur umtalsverðum skaða eða áfalli fyrir mannekjuna. Þessi skaði getur verið á félagslega sviðinu (t.d. síendurtekin afbrot andfélagslegs persónuleika) eða því líkamlega (næringarleysi hins þunglynda eða þess sem er með átröskun). Það kæmi mér því ekki á óvart að í framtíðarútgáfum DSM verði t.d. sá hópur sem nær sér alls ekki undir 40 í LÞS (BMI) nema í stuttan tíma og hefur skilmerki ósértæks stjórnleysis í áti sem ekki verða skýrð út frá hormónatengdum ástæðum og heldur áfram að þyngjast þrátt fyrir heilsufarsvanda tengdan offitu, verði álitinn með offituröskun. Þetta eru frekar fáir einstaklingar sem uppfylla þessháttar skilmerki og ekki hin almenna offeita manneskja.
